Your First Visit
After making your first appointment with the Arizona Reproductive Institute, you will receive a welcome packet mailed to your home. This packet contains information confirming your appointment/consultation, directions to our state-of-the-art center and important medical forms that will need to be completed prior to your first appointment/consultation. On the day of your appointment, we ask that you arrive 15 minutes before your scheduled appointment time. Please complete and bring the following to your appointment:
- Insurance card and
insurance coverage information
- Driver’s license photo ID
- Completed forms:
- Patient Demographics
- Female History
- Male History (if applicable)
- Medical Record Release
- Insurance co-payment or self payment
- We accept cash, personal check, MasterCard or Visa.
Much of the first appointment is spent reviewing past medical records. We need you and your partner to complete and fax to us your medical record release form at least 10 days prior to your first appointment/consultation. That allows us to obtain your medical history. You may also bring your medical records/information to your first appointment if you cannot access them beforehand.
As you meet with Dr. Mansfield, you and your partner will have the opportunity to ask any questions and address any issues. Fertility concerns are best resolved in consultation with both you and your partner. It might be recommended that you and your partner have additional medical testing to help identify the cause of your difficulty getting pregnant. Particular care will be taken so that only the appropriate tests are conducted, saving both time and expense. If this is the case, we can schedule these during your first visit.
After you have completed your initial consultation with Dr. Mansfield, you will meet with our dedicated financial counselor to discuss cost, insurance coverage and possible payment options tailored to your treatment plan. We work with every patient and couple to find the best financial solution to complement their treatment plan.
